Either your file associations are broken or you don’t have PDF reader.
If you don’t have pdf reader, get it here:http://get.adobe.com/reader/
If you do have pdf reader you will have to fix the associations. If you use windows xp, vista, or 7, right click on the file and go to “open with”. Choose PDF reader if it is in this list. If it’s not, you will have to click on “choose other program” or simliar. You will then have to choose adobe reader in this menu.

Click to select the saved, downloaded application form, hold the Shift key down while right clicking to get a menu and choose Open With then select Adobe Acrobat or your choice of PDF file reader.
Once the file opens, print the application form and fill in the blanks manually with a pen.
